Four assets are converging on the US market. Spravato (esketamine) has been approved since March 2019, with a monotherapy indication added in January 2025 (FDA, n.d.). COMP360 (psilocybin) is under rolling NDA review, with the sponsor guiding submission completion to Q4 2026 and launch to H12027 (Compass Pathways, 2026; Yahoo Finance, 2026). DT120 (lysergide, from Definium Therapeutics, formerly MindMed) posted three positive Phase 3 readouts in 2026 and is guided to file for generalized anxiety disorder in H1 2027 (Pharmaceutical Technology, 2026;Clinical Trials Arena, 2026). TSND-201 (methylone) is in Phase 3 (Otsuka Pharmaceutical, 2026).

‍

Almost everyone is watching the FDA approvals and the DEA rescheduling. Neither decides whether a clinic can afford to open its doors. The decisive documents are a Medicare fee schedule rule finalized in late 2025 for unrelated reasons, and a paragraph of REMS language FDA has not drafted yet.

‍

‍

1. The Four Assets, as of October 2026

‍

Spravato

Status: Approved 2019.

CSA schedule: III already.

Monitored hours per session: 2.

Sessions perpatient-year: 25 to 50.

Monitored hours perpatient-year: 50 to 100.

Code for the hours: bundled in G2082/G2083.

‍

COMP360

Status: Rolling NDA.

CSA schedule: Psilocybin is Schedule I.

Monitored hours persession: about 6 (expected).

Sessions per patient-year: 2 to 4.

Monitored hours per patient-year: 12 to 24.

Code for the hours: 0820T to 0822T, unpriced.

‍

DT120

Status: NDA guided for H1 2027.

CSA schedule: LSD is Schedule I.

Monitored hours per session: 8 minimum in Phase 3.

Sessions per patient-year: 1 to 2.

Monitored hours per patient-year: 8 to 16.

Code for the hours: same, unpriced.

‍

TSND-201 (methylone)

Status: Phase 3.

CSA schedule: Methylone is Schedule I (DEA, 2013).

Monitored hours per session: at least 8.

Sessions per patient-year: 4 per course.

Monitored hours per patient-year: 32.

Code for the hours: same, unpriced.

‍

One correction first, because it cuts against the common framing. Methylone is non-hallucinogenic, which is widely read as meaning a light delivery model. The published trial methods state that each dosing session "lasted at least 8 hours or until all study drug effects subsided," four weeks running (Jones et al., 2026). That is roughly 32 monitored hours per treatment course, the heaviest profile of the four and more than double COMP360's.

‍

‍

2. Where Each Claim Gets Stuck

Spravato is covered and still gets denied. Prior authorization is near-universal and routinely granted. The claim fails in adjudication, after the clinic has already bought the drug at roughly $834 for a 56 mg session andup to $1,319 for 84 mg (Neolytix, 2026; AtaiBeckley, 2026). The named denial grounds are specific: authorizations that cover a block of sessions and lapse mid-course, and REMS monitoring forms filed outside the required window (SPRAVATO REMS, n.d.).

‍

COMP360 clears threegates and fails on the fourth. DEA must schedule the product within 90 days of the later of the HHS recommendation or the approval notice (21 U.S.C. § 811(j), n.d.). States then move. Payers then cover thedrug, which they will. Then somebody has to pay for six hours of monitored chair time, and no priced code does. A level-5 office visit spread across a six-hour day is about $32 per chair-hour.

‍

DT120 has the same problem with worse arithmetic. Eight hours, two staff, a room for theday, for one annual dose, so no repeat volume across which to amortize the billing expertise a hard claim requires (Drug Discovery& Development, 2026).

‍

TSND-201 has no claimyet. Phase 3 is recruiting; there is a Breakthrough Therapy designation for PTSD but no NDA (Psychiatric Times, 2025).

‍

‍

3. The Rate at Which a Clinic's Math Works

The only published cost figure for monitored psychiatric chair time is $200 to $400 of staffing and facility overhead per two-hour Spravato session (Neolytix, 2026), which is $100 to $200 per chair-hour. Applying that unchanged to the longer sessions is generous, since they require dedicated rather than shared staffing. On that basis a six-hour COMP360 session needs $600 to $1,200 of service payment before any margin and before the drug. Spravato needs only $34 to $66 per chair-hour, because roughly three patients share an observation bay.Concurrency is the whole trick.

‍

What is billable today, per monitored chair-hour:

Category III codes 0820T to 0822T: no RVU, no national rate (BrainFutures, 2023).

Level-5 office visit alone (99215, $192.39) over the session: $24 to $32 (medfeeschedule,2026).

Spravato bundle, net of drug acquisition: $19 to $178.

Office visit plus every prolonged unit honored (99215 + G2212): $129 to $162 (ClaimMax RCM, 2026).

‍

The last line looks like the answer. It is not, for a reason visible only once the physician's time is charged to the cost side. Prolonged-service codes bill the physician's own time on the date of encounter. One physician cannot bill twenty units of their own time against six simultaneous patients. That path is therefore confined to one patient per physician per day.

‍

Price that configuration. At $30 an hour for the room, $150 for a loaded physician and $45 for a loaded in-room monitor (illustrative figures, but the shape survives substitution), a six-hour session costs about $1,350 with the physician in the room plus an assistant, or $1,620 with two staff in the room. Revenue is $792 to $972. The priced path loses $378 to $828 per patient-day under every configuration, because $132 to $162 per physician-hour against a loaded physician-hour of roughly $150 pays for the physician and nothing else.

‍

The alternative is an hourly clinical-staff service, which one physician can superviseacross several rooms.

‍

Break-even payment per monitored chair-hour, by number of rooms one supervising physician covers:

‍

One monitor in the room.               Two staff in the room:

1 room:                 $270                                                       $225

2 rooms:               $195                                                       $150

3 rooms:               $170                                                       $125

4 rooms:               $158                                                       $112

6 rooms:               $145                                                       $100

8 rooms:               $139                                                       $94

‍

At two rooms the required $150 sits inside what the priced path pays. At six it falls to $100. The configuration that works financially is the one the priced code forbids, and the code that permits it has no price.

‍

What makes the right-hand columns legally available is a change almost nobody in this field has noticed. CMS's CY2026 Physician Fee Schedule final rule permanently adopted a definition of direct supervision satisfied by real-time two-way audio and video, effective 1 January 2026, covering services furnished incident to a physician's service under42 CFR 410.26; behavioral health services may run under general supervision by auxiliary personnel, and the supervising practitionerneed not be the treating practitioner (Centers for Medicare & Medicaid Services, 2025; Morgan Lewis, 2026). It was made permanent in a rule whose discussion was framed around diagnostic tests.

‍

Set that beside what FDA requires. The esketamine REMS states the drug is administered "under the direct observation of a healthcare provider" and that patients are "required to be monitored by a healthcare provider for at least 2 hours" (SPRAVATO REMS, n.d.). Healthcare provider. Not physician.

‍

The two requirements are compatible, and that compatibility is the business model: a provider in the room, a physician on video, several rooms at once. Spravato already runs a version of it. A REMS that instead named a physician, required physical presence, or specified one-to-one staffing would delete the only viable column in Table 3. FDA would be writing that language on safety grounds, with no mechanism for considering any of this, and REMS terms are not fixed at approval: esketamine's has been modified ten times since 2019 (FDA, n.d.).

‍

The fix is narrower than people assume. Not a coverage decision and not an FDA decision, but a priced hourly code for clinical-staff monitoring under physician direction: 0822T with an RVU, or a purpose-built G-code of the kind CMS created for esketamine in January 2020.

‍

‍

4. Nobody Owns the Gap, and Nine to Twelve Months Is the Wrong Number

The gap between approval and payment is eight handoffs: FDA's label, FDA's REMS, DEA's scheduling rule, the states, CMS on benefit category, and code pricing, the AMA panel and RUC on code valuation, the Medicare contractors on self-administered drug lists, and the plans on allowed amounts. The manufacturer has the strongest incentive to close it and authority over none of it.

‍

Three different gaps get collapsed into one estimate. Approval to legal sale is three to five months: Epidiolex took 95 days to its DEA rule and 129 to launch (Food and Drug Law Institute, 2018). Approval to site readiness is where nine to twelve months is genuinely sourced, and it is sourced to a different drug: Sage told the SEC that most interested settings would need "nine months or longer" to become ready to infuse Zulresso, and large systems "12 months or longer" (Sage Therapeutics, 2020). Approval to a durable paid claim is longer than either, because it waits on a priced code, which Compass puts at roughly 18 months after launch (Yahoo Finance, 2026).

‍

‍

5. What Payers Will Cover, and What They Will Leave

Payers cover molecules and resist hours. The asymmetry is structural: the benefit has machinery for pricing a new molecule and none for pricing a new unit of clinician time.

‍

Spravato is already must-cover, at roughly $1.7 billion a year (Bloomberg, 2026). COMP360's drug becomes must-cover quickly in treatment-resistant depression, and its six hours stay with the clinic. DT120's drug gets covered for large non-refractory populations where cheap alternatives exist, which is precisely why its eight hours get left harder. Methylone gets left entirely; Otsuka's acquisition of Transcend for up to $1.225 billion buys the balance sheet to subsidize delivery, and that subsidy will stand in for coverage (Otsuka Pharmaceutical,2026).

‍

Capacity is not the constraint. A dedicated room runs about 250 sessions a year, so 62 to125 COMP360 patients, against roughly 8,500 interventional psychiatry sites (HealingMaps, 2026). The constraint is which operators can run a one-patient-per-room day at a collectible rate while fronting the drug. About 70% of directory-listed ketamine clinics never got onto the Spravato rail even with a priced Medicare bundle and seven yearsof coverage history (HealingMaps, 2026).

‍

‍

6. One Question for a Payer Medical Director

Not "will you cover it," which gets a yes with prior authorization and tells you nothing.

‍

When COMP360 is approved, will you pay a site separately for the six monitored hours as an hourly service, or do you expect them inside an office-visit code? If hourly, name the per-hour allowed amount you would put in a2027 contract.

‍

An hourly rate of $150 to $200, adjudicated without unit-level review, and the long tail of sites can open. A case rate above roughly $900 per session is better than it sounds, because it removes unit-level denial. "Inside the E/M," and the first two years look like Spravato's first two years. "Under the pharmacy benefit," and the drug gets paid while the room never does.

‍

A note on the numbers. Approval dates and the REMS history come from the Drugs@FDA record rather than press coverage; the two differ by days. The break-even band rests on a single published cost figure, and the room, physician and monitor hourly rates in the concurrency analysis are illustrative: directions of effect survive substitution, specific room counts do not. Dollar figures per dose are my arithmetic on the sponsors' own annual-cost guidance, not sponsor-stated prices. A full claim-by-claim source register is available.
